To maintain a clean, healthy, and low-risk environment for all our guests, we ask every owner to complete this Vaccination & Health Declaration form honestly and in full. This simple step helps us ensure that every dog can play, rest, and thrive together without unnecessary risks.

We understand that gathering records and filling out forms can feel like a bit of extra work—thank you for your patience and cooperation! It's because of responsible pet parents like you that we can uphold the high standards that make The Bark Park a trusted place to leave your loved one.

Your dog's safety is our top priority. While our team carefully supervises all playgroups and works to create a safe, positive environment, dogs are animals and play can occasionally escalate in a matter of seconds. Minor scrapes, bumps, or other play-related injuries can happen despite close supervision. By participating in group play, you acknowledge and accept these inherent risks.

Please note: For the safety of every pup in our care, we do not make exceptions to our vaccination and health requirements. This firm policy allows us to keep our facility strong, sanitary, and secure—so you can relax knowing your dog is in the best possible hands.
